To register for our ball hockey tournament taking place Sept 10 at Eleanor Pew Memorial Arena in Chester, please complete this Google Form. To complete the form you will need all players birth dates and emergency contact information. 

Please ensure each player brings with them the day of the event a completed and signed waiver by a parent or legal guardian.
